  • Non-Surgical Treatment | Harlow | Mr Mahaluxmivala

Explore our comprehensive non-surgical treatment options for managing pain and improving mobility. Learn about physical therapy, medication, lifestyle changes, and other innovative therapies designed to help you achieve optimal health without surgery. Non-Surgical Treatment, Orthopaedic Counselling & Physiotherapy Non-surgical treatment and orthopaedic counselling are often recommended for patients who do not require surgical treatment. These modalities are as vital as “high tech” surgery. Physiotherapy is used in The immediate post-operative period of an operation. Knee injuries like ligament and soft tissue injuries not needing surgery All anterior cruciate ligament ( ACL) injuries. Patella dislocations of subluxation, especially after the first and second episodes. Non-traumatic knee conditions like patella tendinitis and quadriceps tendinitis. These often benefit from local utrasound treatment as well. Osteoarthritis of the knee which is not severe enough for an operation may benefit from targeted sessions of knee physiotherapy to strengthen muscles. However, results of physiotherapy for osteoarthritis of the hip are not as favorable as for other joints. Nevertheless, if part of the pain pattern is related to a back problem, then physiotherapy can be beneficial. Local ultrasound treatment can also be used to assist in cases of trochanteric bursitis and snapping iliotibial band (snapping or popping hip) Orthopaedic Counselling: As a high-volume joint replacement surgeon with extensive experience, Mr. Mahaluxmivala understands that optimal patient selection is crucial for achieving the best surgical results. High-risk and overweight patients may still benefit from joint replacement surgery, provided they are fully informed about the risk-benefit ratio. Therefore, Mr. Mahaluxmivala accepts these patients after ensuring they have been adequately counselled and all conservative or non-surgical treatments have been exhausted. The clinic advocates for: General counselling on "joint health," advising patients to lose weight if overweight and engage in regular activities such as walking, cycling, swimming, and traditional gym workouts. Exploring non-surgical treatments such as osteopathy, acupressure, and acupuncture, if surgery is not an option. The clinic emphasises respecting other specialties to maximise patient care. Encouraging patients to join gyms and work with personal trainers to ensure that exercises to strengthen muscles around the knee are performed correctly. Correct form and sustained muscle-strengthening regimes are crucial for addressing lower limb and back pain issues. For patients in the Hertfordshire and Essex areas, the clinic can recommend personal trainers to assist with these efforts. Book an Appointment

  • Knee Arthroscopy | Harlow | Mr Mahaluxmivala

"Learn about Knee Arthroscopy, a minimally invasive procedure used to diagnose and treat knee issues. Knee Arthroscopy 01 What is Arthroscopy? Arthroscopy is keyhole surgery used to diagnose and treat problems within the knee joint. Arthroscopy equipment is small, so only small cuts in the skin are needed. This has the advantage of less pain after the operation, faster healing time, and lower risk of infection. 03 Symptoms The best outcomes for this surgery are typically seen when the patient reports a clear history of twisting or injuring the knee, which triggered the symptoms. Key indicators include: Primary complaints of locking and giving way (mechanical symptoms) rather than constant pain. Pain localised to the inner or outer aspect of the knee, sometimes felt deep within or all over the knee. Pain and acute catching or locking worsen when walking on uneven surfaces or twisting the knee. Swelling and inflammation in the knee may be present initially when symptoms begin. Patients are often normally active and sporty, with no X-ray evidence of arthritis in the knee. 02 Causes The procedure is used to treat a number of conditions, including tears of the cartilage, (meniscus), removing fragments of loose bone or cartilage, drilling of bone to promote healing of cartilage and treatment of an anterior cruciate ligament (ACL) tear. The most common reason for knee arthroscopy surgery done by Mr Mahaluxmivala is a torn meniscus (shock absorbing cartilage) of the knee. Both the cartilage on the inside and outside of the knee can be torn and both can be suitable for knee arthroscopy. Mr Mahaluxmivala explains however that this is one of the operations where “patient selection” is vital for this surgery to have a successful outcome. Insurance companies have strict criteria for allowing this surgery to be performed and this is done to make sure unnecessary surgeries do not occur. 04 Diagnosis Diagnosis involves a detailed history and thorough examination. It is confirmed with: An X-ray of the knee, including three specialised views, such as standing views. An MRI scan to detect cartilage tears, ligament issues, or arthritis not visible on X-rays. Mr. Mahaluxmivala will review the patient's X-ray and MRI images during the consultation, explaining the issues and the need for knee arthroscopy.

  • Revision Total Knee Replacement | Harlow | Mr Mahaluxmivala

Learn about Revision Total Knee Replacement surgery, a procedure to replace or repair a failed knee implant. Discover symptoms, treatment options, recovery process, and how our expert surgeons can help restore mobility and relieve pain. Revision Total Knee Replacement 01 What is Revision Total Knee Replacement Surgery? Revision Total Knee Replacement surgery involves replacing a previous knee replacement with a new artificial joint. It's a complex procedure, more intricate than the primary (initial) total knee replacement (TKR). Revision surgery is typically more complicated because the surgeon must remove the original implant, which has integrated into the existing bone. Additionally, there is often less bone remaining after the prosthesis is removed. Therefore, the procedure requires meticulous preoperative planning, specialised tools and implants, and greater surgical skill. It also takes longer to perform compared to a primary knee replacement. 03 Symptoms Revision knee replacement surgery is not offered solely due to pain; there must be a valid reason for undergoing major re-do surgery. Patients experiencing pain after knee replacement should be thoroughly examined by a knee revision specialist to determine the appropriate course of action. Key indicators for revision knee replacement surgery include: Pain in the knee joint, often with limited walking distance and possibly night pain. Confirmation that the pain originates from the knee joint, not from the hip or back. Complaints of the knee "giving way or collapsing." Swelling, inflammation, and reports of increased knee deformity. Accurate patient selection is crucial for a successful outcome in revision knee replacement surgery. Therefore, a complete history of complaints and a thorough examination of the previously operated knee, hip, and back are essential. 05 Treatment Revision surgery involves the careful removal of the old implant and the placement of a more complex implant designed to compensate for any bone loss. This complexity increases the risks associated with the surgery, and the outcomes may not be as favorable as those of primary surgery. However, advancements in instruments, implants, and surgical skills aim to restore the patient to the results of a successful primary surgery. The actual surgery typically lasts about 120 minutes. However, Mr. Mahaluxmivala advises relatives not to expect the patient back in the ward within this short time frame. This is because a significant amount of care and time is devoted to preparing the patient for anaesthesia and ensuring safe and correct positioning before surgery. 02 Causes The primary reason for revision knee or replacement surgery is often loosening of the previous implant. This loosening may be caused by infection or wear and tear over many years. Other reasons for revision knee replacement surgery include misalignment or instability. A less common but significant reason for revision knee replacement surgery is fracture around the original knee replacement (periprosthetic fracture). 04 Diagnosis Diagnosis of issues with the original total knee replacement involves: A thorough history and complete examination. Confirmation via X-ray of the knee, including three specialised views, including standing views. Periodic specialised X-rays may be needed to monitor for evidence of loosening. In some cases, a radioisotope bone scan or a CT scan may be necessary, typically performed 18 to 24 months after the initial knee replacement. Blood tests may also be required. Aspiration procedure to remove fluid from the knee, which is then sent to a lab for analysis to determine the type of infection and the need for revision surgery. This procedure is done in the operating theatre without the need for general anaesthesia.
